€5,000 Critical Illness/ Personal Accident/ Illness Evacuation or Repatriation Expenses:
- Evacuation or repatriation expenses cover up to €250,000 for members deceased, seriously injured or ill abroad and requiring repatriation to their country of residence

Benevolent grants are available to members experiencing financial distress. To apply, please complete this fillable PDF application form and send it to your branch officials for completion of page 6 (branch comments). Once complete, the form should be submitted to benevolentfund@forsa.ie.
