Deduction at Source (DAS) application form

Bank Standing Order application form (BSO)

Use this form if your employer does not provide an option to have your Fórsa subscription deducted from your salary. New Fórsa members – Get 9 months’ FREE Salary/Income Protection! See application form for details.

Change of Employment (DAS)

Use this application form if you are already making a payment to Fórsa via DAS, and where your new employer also facilitates DAS.

Change of Employment (BSO)

Use this application form if your union sub payment is currently, and will continue to be, made via bank standing order.

Promotion to Executive Officer (EO)

(Civil Service only) To be used solely by an existing Fórsa member transferring to a new employment/payroll section upon promotion to Executive Officer.
